On this page, you can view and/or update company related information This section caters the General Inforamtion sub-section of the Exhibit-B from the service agreement. Incase you have rights to edit company information, click on "Edit Company Information" button and then update the information of interest. Click on Submit to save the changes, click on cancel to go back to the read mode.

• Supplier Company Name: This is a read only field to display the company name.
• Supplier Company Code : This is a read only field to display the company code provided by the supplier relations team of UI.
• Supplier Legal Name : Legal name of the supplier, non eidtable field.
• DPUC Docket(xx-xx-xx) : Iditable and it is in the format of xx-xx-xx(optional). If it is provided then non customer list is available for the supplier admins and corresponding supplier members . Editable field to capture the DPUC docket number, it should be in the format xx-xx-xx. Till the DPUC docket number is not provided here, none of the members of your company registered in UI SMS system, will have access to view/download the Non-Private customer list data.

Corporate Address: This is cater the corporate address of your company.

• Address Line1: Street Address information of the company address. (mandatory)
• Address Line2: Additional address information of the company address. (optional)
• City: The city name of the company address. (mandatory)
• State: The state of the company address. (mandatory)
• Zip: The 5 digit(no post box number) ZIP code of the company address.(mandatory)

• DBA Name : DAB name of the company(optional)
• Type of Business Entity : Business Entity type(optional)
• Tax ID# : Tax ID value(optional)
• Dun & BradStreet # : Dun & BradStreet(optional)
• Customer Service Email id : Customer Service Email(optional)
• Customer Service Phone# : Customer Service Phone number icludes 10 digits icluding are code(optional)
• URL: URL of the company eg www.uinet.com(optional)
• Fax Number: Fax number od the supplier company(optional)
